Note: We do not accept insurance (we do accept cash/credit/debit). We do not perform annual or preoperative physicals.
- Pre employment physicals for work; we also offer PPDs (TB test, $50)
- School attendance physicals (i.e. NYS School Health Examination Form)
- Sports participation physicals (i.e. NYS School Health Examination Form)
- Physicals to obtain licensure (i.e. cosmetology, barbering, esthetics, nail specialty – Health Certification Form)
- Physicals for participation in agility tests (i.e. police officer/sheriff)
- Coast Guard and DOT physicals are $200
- Merchant Mariner/USCG Physical ($200, CG-719K)
Physicals at Surya involve recording medical history, checking vital signs, performing an examination, and completing necessary paperwork. Referrals are most humbly appreciated.

- Walk in with any paperwork that requires completion.
- Complete history portion of Surya’s physical form.
- Make payment of $200 (cash/credit/debit). We do not accept insurance.
- Complete the physical visit.
- Receive the physical form. A provider will sign your paperwork (if applicable).
